To Patients, Clients, Residents, Families, and Visitors

Good communication with your care team is important at all times, and even more so, if you have a concern.

Your health-care team knows you best. If you would like to communicate a concern, please take the following steps:

  1. Because your care team knows you best, first discuss your concern with your team. You may also ask the Manager of the program or service to help you with your concern.
  2. You may also contact the Patient Concerns Department.
    • You may contact us by telephone or mail. Please refer to the listing of contact information. Or;
    • You may complete this online form. A contact person will be in touch with you for on-going inquiries and information sharing within 3 business days.

Our Approach

  • Once we hear from you, a contact person will be in touch with you by phone within three (3) business days.
  • The contact person will clarify details of your concern, and, you will be informed of the next steps that will be undertaken to address your concern.

Patient Concerns Officer

The Patient Concerns Officer (PCO) is appointed by the President & CEO for Alberta Health Services in compliance with provincial regulations. The PCO is responsible for determining administrative fairness of the process used to address a specific concern. After working with the Patient Concerns Department, if you are not satisfied, then contact the Patient Concerns Officer.

The Alberta Ombudsman operates independently from any part of Alberta Health Services.

Privacy and Confidentiality

AHS protects the privacy of individuals receiving health services in accordance with the Health Information Act. To properly review and resolve concerns, we work with the patient, or an authorized representative (usually a close family member), to gather and share information about the services received.
